Mental Health Support in Japan: English-Speaking Therapists, Clinics, and Online Options for Expats
Living in Japan is rewarding, but it can also be quietly exhausting. Language barriers, cultural adjustment, distance from family, overwork culture, and the isolation that can come even in a crowded city all take a toll. Yet many foreign residents in Japan either do not know where to turn or assume that adequate English-language mental health support does not exist.
It does exist. It is fragmented and underfunded, and Japan's health insurance system does not cover all of it — but there are real options. This guide maps what is available, what it costs, and how to access it.
This article is general information, not medical advice. If you are in crisis, please call the TELL Lifeline at 03-5774-0992 (available daily, 9
–23).
Why Mental Health Support Is Harder to Find in Japan
Japan has a shortage of licensed therapists even for Japanese speakers. Mental health care carries significant social stigma in many social contexts, and the psychiatric system is historically hospital-focused rather than outpatient therapy-focused.
For foreign residents, an additional layer: national health insurance (国民健康保険 / 社会保険) covers psychiatric medicine, but does not cover talk therapy or counseling. This means a visit to a psychiatrist who prescribes medication is covered; a session with a psychologist or counselor is paid entirely out of pocket.
Understanding this split is the starting point for navigating the system.
TELL is the oldest and most established English-language mental health organization in Japan. They offer:
Face-to-face counseling in Tokyo (Hiroo office)
Online counseling sessions
A sliding-scale fee structure (fees adjusted based on income, minimum around ¥4,000)
The Lifeline for immediate crisis support
TELL's counselors are licensed professionals, predominantly trained in Western therapy frameworks. Wait times can be several weeks for ongoing sessions.
Tokyo Mental Health
Website: tokyomentalhealth.com
Tokyo Mental Health offers individual therapy, couples counseling, and family therapy in English. Therapists use CBT, ACT, and psychodynamic approaches. Sessions are ¥10,000–¥15,000. They also offer online sessions, which extends accessibility outside Tokyo.
Cross Over Care
A newer provider with therapists in Tokyo and online. Focuses on expat-specific challenges: cultural transition, work-life stress, relationship issues across cultures. English and some bilingual options.
Online Therapy Platforms Available in Japan
If you are outside a major city, or prefer the flexibility of online sessions, several international platforms serve Japan-based clients.
BetterHelp
BetterHelp matches users with licensed therapists and conducts all sessions via video, phone, or messaging. It is not Japan-based, so therapists are predominantly licensed in the US, UK, or Australia. This matters for treatment approach and cultural context, but many users find it effective.
Cost: approximately ¥13,000–¥20,000/month (subscription model, not per-session).
Talkspace
Similar model to BetterHelp. Some users find the asynchronous messaging feature useful for processing thoughts in writing between sessions.
Japan-Based Online Counseling Platforms
Several Japanese platforms now offer English-speaking counselors:
Cotree (コトリー): Primarily Japanese but has English-capable counselors. Lower price point (from ¥4,000/session).
medimee: English available; connects with therapists and psychiatrists.
Support Groups and Community Resources
Peer support and community are underrated for mental health maintenance.
AA Japan (Alcoholics Anonymous): English-language meetings in Tokyo, Osaka, Nagoya, and online. aa-japan.or.jp
TELL Support Groups: TELL runs specific groups for grief, anxiety, and expat adjustment. Free or low-cost.
Internations / Facebook expat groups: Not clinical, but community connection reduces isolation. Many cities have informal support networks.
What to Expect at a Japanese Psychiatric Clinic
If you go to a Japanese-language 心療内科 (psychosomatic medicine) or 精神科 (psychiatry) clinic with limited Japanese:
Bring a translation of key symptoms written in Japanese (Google Translate or DeepL is acceptable for intake paperwork)
Most clinics have intake forms. Some have English versions for international clinics.
Sessions are short by Western standards — typically 5–15 minutes after the first diagnostic visit
The doctor will primarily manage medication; ongoing talk therapy is not standard
Follow-up appointments are frequent (often every 2–4 weeks) to monitor medication response
This model differs sharply from therapy-centered Western mental health care. Managing expectations helps.

Summary
Japan's mental health system was not designed with foreign residents in mind, and national health insurance does not cover counseling. But English-language options exist: TELL for sliding-scale counseling and crisis support, Tokyo Mental Health for private therapy, insurance-covered psychiatrists for medication management, and international platforms like BetterHelp for online access.
The gap is real but bridgeable. Knowing the landscape is the first step.
Frequently Asked Questions
Q: Can my insurance cover any of the counseling costs?
Japanese national health insurance does not cover psychotherapy or counseling. However, if you have supplemental international health insurance (common for corporate expat packages), some policies cover mental health treatment. Check your policy documentation under "mental health benefits."
Q: Is there stigma around seeking mental health treatment in Japan?
Among the Japanese population, stigma remains present in some contexts. Among the international expat community, it is generally much lower. English-speaking therapists in Japan are accustomed to working with clients from cultures where mental health care is normalized.
Q: Can I see a therapist online from Japan with a therapist licensed in my home country?
You can engage with them as a client, and many do. However, therapists in some countries (notably the US) technically require a license in the state where the client is located to practice legally. In practice, this is rarely enforced internationally, but it is worth asking your therapist about their policy.
Q: What if I need an ADHD evaluation or autism assessment in English?
Some international clinics in Tokyo offer neuropsychological assessment in English. Tokyo Medical and Surgical Clinic and some private psychology practices offer this. Expect significant out-of-pocket costs (¥50,000–¥150,000 for a full assessment). Contact clinics directly to ask about availability.
